Your health record

Your GP record is held securely and confidentially on our practice’s electronic system. It includes details like your address, any contact you’ve had with the surgery, notes about your health, and results of tests.

Ways your record can be used to support your care

  • Electronic appointment booking and online repeat prescription ordering
  • GP to GP transfers, if you register with a different practice
  • Patient access to records — viewing your medical record online

Summary Care Record

The Summary Care Record is used across England to give healthcare staff outside this surgery faster, secure access to essential information about you in an emergency or when you need unplanned care. It includes your current medications, allergies and any bad reactions to medicines.

Your permission will be asked before this information is accessed, unless a clinician cannot ask you and there is a clinical reason to access it.

If you would like to opt out, ask reception for an opt-out form. A parent or guardian can request to opt out a child under 16, but it is ultimately the GP’s decision, because of their duty of care to the child.

Viewing or requesting your record

You have a right to see your records. Ask at reception for details of our patient information leaflet — an appointment may be needed, and in some circumstances a fee may apply.

Read our Privacy policy for more about how we use and protect your information.
