Marie Curie Cancer Care is a UK charity dedicated to the care of people with terminal cancer and other illnesses. Over the financial year 2010/11, we reached a total of 31,799 patients

Youth Health Talk

Youthhealthtalk enables young people, their family and friends, and professionals such as doctors and teachers to understand young people's experiences of health, illness and life in general. The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

New Patient Registration

Thank you for your interest in registering at The Kirkbymoorside Surgery. The doctors welcome new patients who live within our practice area.  To find out if you are within our practice area, please contact our Patient Support Team on 01751 431254.

ID Evidence needed for Registration

We take the security of your medical information extremely seriously. To register at the surgery, please print off and complete the Registration form (which you can find by clicking on the tab above that says Registration Form) and bring it with you to the Surgery together with proof of your ID AND also your current address. You must bring one original document from each of the two lists below.

LIST 1:   Proof of ID (must contain a photograph and be current):

  • Passport
  • Driving Licence
  • NYCC parking card for disabled people
  • Citizen Card
  • Bus Pass

LIST 2:   Proof of Address (must be dated within the last three months)

  • Bank of Building Society statement
  • Gas / electricity bill
  • Telephone bill
  • Council tax bill
  • Payslip

If you do not have this evidence then please contact us and we will try to help.

Registering Children

If you are registering a child under the age of 16 then please bring evidence of your link to the child e.g. a birth certificate or a letter of adoption.

New Patient Check

Once your form has been processed, we will ask you to complete the new patient check on our Surgery Pod. 

Needing Medication?

If you have moved to the area and require medication then please contact the Surgery to make an appointment to see a GP. You will be asked to attend 20 minutes earlier than the appointment so that you can be registered first. Please bring the Registration Form with you fully completed and also your ID (see above) as the GP will not be able to prescribe any medication for you without having access to your medical records which requires you to register first.

Online Services

We now offer many services online e.g. ordering repeat prescriptions, notifying us of a change of address, booking all GP appointments. The ID evidence you need to register as a SystmOnline User is the same as that which you need to register at the Surgery so ask about registering for our online services when you come in and it will save you a trip to the Surgery!
