How to Register
If you would like to register at the Surgery and you live within our catchment area then please complete a GMS1 form and return to the Surgery. We cover an area from the Lion Inn in Blakey to the North, Butterwick to the South, Aislaby to the East and Pockley to the West. Please contact the practice if you wish to check a specific address.
We take the security of your medical information extremely seriously. When registering, you will need to bring an official document that contains your photograph (e.g. passport, modern driving licence, current NYCC Parking Card for Disabled People, Citizencard or Bus Pass) and a second official document that can verify your address and which is dated within the last three months (e.g. bank or building society statement, gas / electricity bill, telephone bill, council tax bill or a payslip). If you are registering a child aged under 16 then please bring evidence of your relationship to the child e.g. birth certificate or a letter of adoption. At the point of registration, an appointment will be made for a New Patient Check. This will be with a Health Care Assistant unless you need any medication in which case it will be with a GP.
The right of patients to express a preference of practitioner
Patients are allocated a GP upon registration but may request an appointment with any practitioner at the Surgery.

2. ACCESS TO THE SURGERY
Surgery Opening Times
Monday and Tuesday: 8.00am - 7.00pm (we do not accept telephone calls after 6pm)
Wednesday to Friday: 8.00am - 6.00pm
First Saturday of each month only: 8.15am – 11.15am (Appointments available 8.30am until 10.30am. Please note Saturday morning appointments are pre-bookable in advance only. If you need to see a GP urgently you will need to ring NHS 111)
Appointments can be booked weekdays between 8am and 6pm (excluding bank/public holidays), and on Saturdays between 8.30am and 10.30am.

Access for Disabled Patients
Kirkbymoorside Surgery provides full access for disabled patients. There are two disabled bays available in the Surgery car park.
You will normally be allocated a ten minute appointment. If you have more than one problem to discuss then please let the receptionist know so a longer appointment can be made.
If you are unable to make your appointment then please let us know as soon as possible so that this appointment can be offered to another patient.
Our appointment system allows you to book non-urgent appointments up to six weeks in advance.
Telephone appointments for either a doctor or a nurse can also be made.
If you have an urgent problem then you will be offered one of the following:
An appointment with a doctor that day
An appointment with a nurse that day
Telephone advice from a doctor
Please note that it may not be possible for you to speak to your usual doctor.
If you are unable to get to the Surgery then it may be possible for a doctor to visit you at home. Wherever possible, please contact us before 10.30am if you think you need a home visit.

4. OTHER HEALTH CARE PROVIDERS
Local Walk-In Centre
Our nearest local walk-in medical centre is:

The centre is open from 8.00am – 8.00pm every day of the year.
The telephone number for NHS Direct is 111. You should call the NHS 111 service if you need medical help fast, but it’s not a 999 emergency. For more information about NHS 111 please visit http://www.nhsdirect.nhs.uk/.
Community Services - Supporting People in their Homes
To support our patients in the home, we work closely as a team with the community nurses and health visitors who have a base in our building. We also work closely with the Macmillan, Heart Failure and Respiratory and Diabetes Specialist nurses.

Due to the skills and experience of the doctors and clinical team within the practice, the following enhanced services are offered:

A team of District nurses are also based in the surgery.
6. REPEAT PRESCRIPTIONS
Authorised repeat prescriptions can be re-ordered by telephoning our 24 hour Repeat Prescription Line on 01751 434916.
You can also drop your order form in at the Surgery. Please allow up to 48 working hours for your prescription to be prepared and signed by a doctor.
If you would like to arrange for your prescription to be taken to the chemist in Kirkbymoorside (Towler’s) then please contact us and we will be happy to arrange this for you.

7. PATIENT FEEDBACK
We are always looking for ways to improve the services we offer and are more than happy to hear any suggestions you have. Please use the suggestion box at the surgery, speak to one of our staff or GPs or send us a comment online. You can also join our lively Patient Representative Group called Patient Matters. Patent Matters – putting patients first
Are you interested in what happens at your local Surgery? If so then why not join Patient Matters ? This group will help keep the Surgery in touch with what its patients have to say about everything that matters in the Surgery.
If you join the group you do not have to come along to the meetings that take place every three months but could just receive a survey to complete once or twice a year. You can choose what level of involvement you want.

SMS Text Messages
The Kirkbymoorside Surgery is now able to contact patients by text if they give us permission to do and have provided a valid mobile number. Please complete the SMS Text Messaging Consent Form if you would like to receive reminders about appointments, follow-up reminders about any annual reviews that we have written to you about and asked you to make an appointment for, as well as other ad hoc messages containing personal sensitive medical information about you. Text messages will appear from firstname.lastname@example.org.

9. RIGHTS AND RESPONSIBILITIES OF PATIENTS
Rights and responsibilities of the patient
Patients can expect to be treated with respect at all times and to receive a high standard of care from the practice. Click here to see our Patient Privacy and Dignity Policy.
Please note that the doctors recognise that all of their staff work hard to ensure the smooth running of the practice and provide a comfortable and safe environment for patients to be in. Please treat all our staff with the courtesy and respect they all deserve.
Action taken against violent patients
Physical violence, verbal abuse or intimidation against any of the Surgery staff or GPs will not be tolerated and is likely to lead to a patient being removed from the Surgery list.
Access to patient information
Everyone working in the NHS has a legal duty to keep information about you in accordance with the Data Protection Act 1998, the Human Rights Act 1998 and the common law duty of confidence. The leaftet How we use your medical records provides more information about access to patient information.
Patients have the right under the Freedom of Information Act to ask for a copy of their records. All requests for information under the Freedom of Information Act 2000 must be made in writing to the practice and a charge may be payable in advance. Proof of ID is also required when the request is made. The practice then has a maximum of 21 days to provide the information requested. If you would like to have copies of all or some of your medical records then please contact us.

The Surgery is proud to be an approved practice for fully qualified doctors who have been working in hospital medicine to come and train in General practice. Please regard them as one of us.
11. VOLUNTARY SECTOR LIAISON TEAM
Voluntary Sector Liaison Team - working with our voluntary and community colleagues to promote healthy lives
In 2010 we created a Voluntary Sector Liaison Team within the Surgery. This team works closely with our third sector colleagues to help promote the essential work that they do both within the Surgery and, where appropriate, with individual patients too. If you are involved with a voluntary or community group and would like us to promote the work that they do then please contact us and ask to speak to Gilly Allanson (Senior Medical Receptionist).
